Massachusetts Department of Public Health Requirements for School Attendance

• Up-to-date immunization record 
•  All children in grade 7 must have three (3) doses of Hepatitis B; two (2) doses MMR; four (4) doses of TDAP  and two (2) doses of varicella vaccines. 

• Physical examination:
• all children entering grade 7
• all children new to the school system
