Identification (all)
- Photo identification of Parent/Guardian
(Driver's license, passport, Massachusetts ID, military card) - Child's birth certificate
- Massachusetts School Health Form
- Physical Health Exam, signed by a physician within the last 12 months
- Immunization record
- Pediatric lead test
- Prior school records (Grades 1-12) - or signed Records Release Request, available from the forms library to left
If applicable
- Massachusetts Transfer Card
- Special Education Document (Individualized Education Plan/IEP or 504 Plan)
- Legal guardianship documentation
- Proof that adult is seeking guardianship
Evidence of Residency (submit ONE from the options below)
- Copy of Deed and/or record of most recent mortgage
- Copy of Lease AND record of most recent rent payment
- Legal affidavit from landlord confirming tenancy
- Section 8 agreement
- Affidavit of Residency
- Massachusetts Driver's License or state identification with Nantucket address
Evidence of Occupancy (submit ONE from the options below)
- Utility bill (i.e. gas, electric, oil, water, sewer, home telephone, cable)
- Payroll stub
- Bank or credit card statement
- W2
- Tax bill (i.e. property or excise)
- Current vehicle registration
- Insurance bill (i.e. home, rental, health)
- Letter from approved agency (i.e. Social Security, WIC, SNAP, disability)
- Photo identification of Parent/Guardian
