Official documents that you must bring to the registration meeting at your school:
Ο Photo ID of parent or guardian with current address (Drivers License, passport, MA ID, military card)
Ο Child's Physical Health Exam, signed by a physician within the last 12 months
Ο Child's immunization record, including lead test
Ο Prior school recordsIf applicable:
Ο Massachusetts Transfer Card
Ο Special Education (Individual Education Plan or IEP, 504 Plan)
Ο Legal guardianship documentation (copy of mittimus or court/agency papers)
Ο Caregiver Affadavit
Ο Proof that adult is seeking guardianship
Ο Records Release Form - if a student attended a previous schoolEvidence of Residency (one document required)
- Copy of Deed and/or record of most recent mortgage payment
- Copy of Lease AND record of most recent rent payment
- Legal Affadavit from landlord confirming tenancy
- Copy of Section 8 Agreement
- Affidavit of Residency
- Driver's LicenseEvidence of Occupancy (one document required, must be dated within the last 60 days)
- Gas bill
- Electricity, oil bill
- Water/sewer bill
- Home telephone bill
- Cable TV/ Internet bill
- Letter from approved agency (Social Security, WIC, Disability, etc.)
- Payroll stub
- Bank or Credit Card statement
- W2
- Excise tax bill
- Property tax bill
- Current vehicle registration
- Home or rental insurance bill
